Thank you for taking the time to take part in our Accessibility Improvement Survey! We are committed to creating a more inclusive community in Yarmouth, and your feedback is essential in helping us assess the progress we've made since implementing our accessibility plan three years ago. This survey aims to gather your insights on what improvements you’ve noticed, what challenges remain, and how can we continue to enhance accessibility for all residents and visitors. Your responses will directly inform our efforts in refreshing our accessibility plan. The survey is located below, and will take approximately 5–10 minutes to complete.

Prefer to complete a paper copy?

Pick one up at one of the following locations:

  • Yarmouth Town Hall
  • Mariners on Main
  • Western Counties Regional Library
  • Yarmouth Hospital (Information Desk in main lobby)
Also, a PDF version of the survey is available for download if you wish to complete the survey by hand. Please deliver your completed survey to Yarmouth Tow Hall, 400 Main Street by 4:30 pm on Friday, February 21st
